1. Find the Building
Look for the white building with the Kurt Thompson Law Group sign. The address number, 2201, is on the side of the building. Park in the lot out front and head inside.
2. Go to Suite 202
Cognitiva’s office is inside Mindful Behavioral Health, Suite 202. You will see Dr. Ivan Cichowicz’s name on the sign next to the door.
3. Check In at the Front Desk
Go to the front desk and confirm your appointment with Dr. Cichowicz / Dr. C.
