These Terms of Service (“Terms”) govern your access to and use of cognitivabrainhealth.com and related tools, forms, waitlist submissions, webinars, and program enrollment (collectively, the “Services”) provided by Cognitiva Brain Health (“Cognitiva,” “we,” “us,” or “our”). By using the Services, you agree to these Terms.
1. Nature of Services
Cognitiva provides education, monitoring, and prevention-focused programs related to brain health, offered at different levels of guidance from self-paced education to physician-guided care. Content on this website is for general informational purposes only and does not constitute medical advice. Use of the website alone does not establish a provider-patient relationship. A provider-patient relationship is formed only after completion of intake, informed consent, and confirmation by Cognitiva.
2. Eligibility
You must be at least 18 years of age to use this website or enroll in a Cognitiva program on your own behalf. By using the Services, you represent that you meet this requirement.
3. Program Enrollment
Cognitiva offers multiple program tiers (Essential, Plus, Personalized, and Concierge). Enrollment, billing, and cancellation terms for each tier are described at the time of signup. Cancel before renew date so as to not be charged. No refunds unless approved by the team.
4. Virtual and Physician-Guided Sessions
Certain programs are delivered virtually, including group sessions and physician-guided care where applicable. By participating, you acknowledge that virtual care involves electronic transmission of information, that there are benefits and limitations to remote care, and that you are responsible for having appropriate technology and a private environment for your sessions.
5. SMS and Text Messaging
If you opt in to receive text messages from Cognitiva, you consent to receive communications such as appointment reminders and program updates. Message and data rates may apply. You may opt out at any time by replying STOP.
6. Communications
Electronic communications, including email, text messages, and any online portal, are used for administrative and program-related purposes. Do not use electronic messaging for emergencies. In an emergency, call 911 or go to the nearest emergency room.
7. Privacy and Health Information
Your use of the Services is subject to our Privacy Policy. Health information collected as part of program enrollment is protected in accordance with applicable law.
8. Acceptable Use

We may suspend or terminate access for violations of these Terms.
